Monochorionic twin gestation is a high-risk type of multiple pregnancy characterized by a shared placenta with intertwin vascular anastomoses, leading to hemodynamic imbalance. Major complications include twin-to-twin transfusion syndrome (TTTS), twin anemia–polycythemia sequence (TAPS), selective intrauterine growth restriction (sIUGR), and twin reversed arterial perfusion (TRAP) sequence. This article summarizes current data on their pathophysiology, diagnosis, and management, highlighting the role of ultrasound and Doppler monitoring and modern fetal therapies that improve perinatal outcomes.
